Insurance & Payment

Your insurance coverage is a contract between you and your carrier. As a service to you, we will file the appropriate reimbursement claim forms with your insurance carrier. To help control the cost of providing medical care, please pay any required co-payments or remaining balances at the time services are rendered. If your insurance carrier fails to provide payment for your services within a reasonable amount of time, we will contact you for payment.

We cannot examine an HMO or PPO patient without a referral. If you are a member of an HMO or PPO, we will need your referral number and/or authorization for treatment prior to your initial visit.
